Can a Toothache Cause an Earache? The Curious Case of Referred Pain

It happens more often than you think.

Someone walks into my clinic, holding their ear. The ache is sharp. They are sure it is an ear infection. But after a careful check, the ear looks healthy. No redness. No infection.

So, where is the pain coming from?

A quick chat and some gentle questions later, we find the answer : a hidden dental problem.

This is something I see regularly. Not every earache starts in the ear. Sometimes, pain travels. We call this referred pain, and it can be confusing, but it is very real.

What Is Referred Pain?

Sometimes, pain plays tricks. It starts in one place, but you feel it somewhere else entirely.

That is what we call referred pain.

In the case of your ear and teeth, it is all about how the nerves work. The nerves that carry signals from your teeth, jaw, and face often share the same pathways with those from your ear.

Think of it like roads merging into a busy highway. Messages from different parts of your head travel along the same path. When there is a traffic jam, like pain from a tooth -the brain sometimes misreads where exactly that signal came from.

So, even though the problem is with your tooth, your brain may register it as ear pain.

This is why people often feel confused. “My ear hurts, but nothing seems wrong with it.”

It is not in your head. It is how the body naturally processes mixed signals. And once we know this, we can trace the pain back to where it really began.

How a Toothache Can Lead to an Earache

It surprises many people, but your teeth and ears are closer neighbours than you think.

Your back teeth , especially the lower ones, sit very near the jaw joint, which we call the TMJ. Right next to that joint is your ear canal. Everything is tightly packed in this small area.

So, when there is trouble with your tooth or jaw, the signals do not always stay in one place. The same nerves that carry messages from your teeth also connect to your ears. This is why pain can easily travel.

I see this often. Someone comes in, worried about an ear infection. But when we check the ear, everything looks normal. No redness. No swelling. Instead, a dental scan shows a deep cavity or an infected wisdom tooth quietly causing the pain.

Grinding your teeth, dental infections, or even impacted teeth can all do this. The ear ends up feeling sore or heavy ; but in reality, it is only reacting to what is happening nearby.

In simple words, the ear is not the problem here. It is just caught in the middle.

When It’s the Ear, Not the Tooth

Sometimes, yes, it really is the ear.

If you are dealing with blocked feelings, crackling sounds, or your hearing feels dull, that usually points to something going on inside the ear itself.

It could be something like fluid build-up, or a mild infection, or even a little trouble with the jaw joint nearby.

When patients tell me, “My ear feels full,” or “I hear popping now and then,” ,  these are small but useful clues. They help us look in the right direction.

So while tooth issues can send pain towards the ear, your ear can also make its own voice heard when something is wrong.

That is why I always say : we need to check carefully. Ears, teeth, jaw-  they all sit close and sometimes confuse us. Listening closely, and examining gently, tells us where the real problem is.

So, Who Should You See First?

I get this question almost every week.

“Should I go to the dentist or come here first?”

My answer is simple.

If it is more about your teeth, like pain when chewing or recent dental work : start with your dentist. They know teeth best.

But if you feel blocked ears, odd sounds, or any discharge, come to me. That points more towards an ear issue.

Many times, I work hand-in-hand with dentists. We often chat about shared cases because sometimes pain likes to blur the lines.

At the end of the day, you do not need to worry about choosing perfectly. Whoever you see first, we will figure it out together. What matters is starting the check. That alone brings you much closer to relief.

Final Thoughts

Pain never shows up for no reason.

If your ear hurts, or if something feels odd , even if it comes and goes : it is worth checking. Sometimes, it is a simple tooth issue. Sometimes, the ear itself needs care.
